For years, weight loss advice came down to eating less and moving more. That advice is still sound, but for many people it was never enough on its own. Hunger, cravings and the body’s push to regain lost weight work against even the most committed effort.
GLP-1 medications have changed that conversation. They work with the body’s own appetite signals, and in clinical trials they helped people lose much more weight than lifestyle changes alone. They have also raised plenty of questions about how they work, which one is best, and how to use them safely.
What Are GLP-1 Medications?
GLP-1 stands for glucagon-like peptide-1, a hormone your gut releases after you eat. It helps control blood sugar and tells your brain you are full.
GLP-1 medications for weight loss work by acting like this hormone. Some were first approved for type 2 diabetes, and some are FDA-approved for chronic weight management. The two you hear about most are:
Semaglutide, sold as Wegovy for weight management and Ozempic for type 2 diabetes
Tirzepatide, sold as Zepbound for weight management and Mounjaro for type 2 diabetes
Tirzepatide acts on two gut hormone pathways, GLP-1 and GIP, while semaglutide acts on GLP-1 alone. Both are usually given as a weekly injection.
How Does GLP-1 Work for Weight Loss?
These medications help in a few connected ways:
Lower appetite. They act on the parts of the brain that control hunger, so smaller portions feel satisfying.
Slower digestion. Food leaves the stomach more slowly, so you stay full longer after a meal.
Fewer cravings. Many patients describe less “food noise,” meaning less constant thinking about food.
Better blood sugar control. They help the body release insulin when blood sugar rises.
The medication does not replace better eating and more activity. It makes those changes easier to stick with.
What Results Have Trials Shown?
When the FDA approved Wegovy in 2021, it reported that adults in the largest trial lost an average of 12.4% of their body weight compared with placebo. When it approved Zepbound in 2023, it reported that adults on the highest dose lost an average of 18% compared with placebo over 72 weeks.
Those are averages from people who stayed on treatment with regular follow-up. Some people lose more, some lose less, and some do not tolerate the medication.
Which GLP-1 Is Best for Weight Loss?
There is no single best GLP-1 for everyone. Tirzepatide showed larger average weight loss in its trials, but the right choice depends on:
Your health history, including diabetes, heart disease, sleep apnea or past pancreatitis
How you tolerate it. Nausea, diarrhea, vomiting and constipation are common, especially when starting or raising a dose.
Cost and access, which vary widely by insurance plan and pharmacy
This is a decision to make with a provider who knows your full medical history.
Who May Be a Candidate?
FDA-approved weight management medications like Zepbound are for adults with:
A BMI of 30 or higher, or
A BMI of 27 or higher plus at least one weight-related condition, such as high blood pressure, type 2 diabetes or high cholesterol
They are not right for everyone. People with a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia syndrome type 2 (MEN2) should not take them. People who are pregnant or planning a pregnancy need a different plan.
Why Supervision Matters
GLP-1 medications work best with steady medical follow-up. Good care includes:
Baseline labs before starting
Slow dose increases to limit side effects
Regular check-ins to track progress and adjust the plan
Nutrition guidance, especially enough protein, to help protect muscle
A long-term plan, since many people regain weight after stopping
Your primary care provider is well placed to do this. They already manage your blood pressure, blood sugar and other medications, and can see how weight loss affects all of them.
Medical Weight Loss at Rise Up Direct Care
Rise Up Direct Care offers a medical weight loss program as part of primary care, including GLP-1 medications when they are appropriate for you. It is a structured, monitored plan that starts with an evaluation and includes baseline labs.
Our weight management pricing is $99 a month, or $297 for a three-month Kickstart option. Ask us what the medication itself would cost in your situation, since that depends on the drug and your coverage.
